Your HBP Won’t Be Replacing Your SOFHA Family Physician By having a HBP, don’t worry that you’re being disloyal to your usual SOFHA doctor or a specialist you’ve had for years. While you’re hospitalized, the HBP, your primary care physician and any other specialist involved will become part of your larger, on-site healthcare team, interacting regularly on your behalf. Hospital based physicians are trained to manage chronic illnesses and conditions; and, due to their constant presence in the hospital setting, can respond quickly to complications. Your attending HBPs will be given access to your medical history at the time of your admission through a highly encrypted electronic environment. Upon your discharge, those records will be returned with updates to your SOFHA primary care doctor as well as to any specialty physician who was involved during your hospital stay. HBPs are doctors or advanced practitioners, most often trained in internal or family medicine, who serve the rotating patient population in a hospital – instead of having a personal practice. They will partner with all other medical professionals at the hospital and also with any other physicians involved – such as your own SOFHA primary care doctor – to ensure you achieve the best possible outcome during your hospital stay. What Will My Hospitalist Do? If you’ve just been admitted to the hospital, then you’ll soon be joined by a Hospital-Based Physician (HBP) provided through State of Franklin Healthcare Associates (SOFHA). He or she can: Answer your and your family’s questions or concerns Prescribe tests and treatment Help plan your treatment after you get discharged from the hospital 5 Quick Facts About SOFHA HBPs They’re part of a new medical specialty known as “hospital medicine,” which is designed to improve healthcare delivery in hospitals. SOFHA provides HBPs around the clock at local hospitals, so our patients receive efficient and timely delivery of care at no extra cost. Your HBP does not replace your regular primary care physician, but rather partners with him or her to ensure information is shared and that you get the best possible care. Since HBPs started being used (about 15 years ago), they’ve been shown to reduce the length of hospital stays for patients by about 30 percent on average – in addition to lowering costs and hospital readmission rates. HBPs are highly trained to manage chronic illnesses and conditions and will respond quickly when complications arise.
