Meeting you where you are…

State of Franklin Healthcare Associates is now offering telehealth appointments to our patients.  Telehealth provides our patients with an option to continue to receive the medical care they need in the comfort of their own homes by allowing access to our healthcare services remotely. Our patients can communicate directly with a SoFHA healthcare provider via a computer, tablet or smartphone with a built-in camera. 

As we continue to navigate these trying times with COVID-19 we want to assure  that you have access to your healthcare provider.  We are dedicated to reducing the risk of COVID-19 exposure to our patients while continuing to provide you with ongoing medical care. All of our clinics are open and prepared to help you with your medical needs. We have implemented extra precautions to ensure the safety of all our patients and staff and are following the TN Department of Health and CDC guidelines for maintaining a clean and safe environment. However, we understand some of you may prefer not to visit the office during this time. If you would like to schedule an appointment with your provider for non-related COVID-19 symptoms, please contact your office directly or click the button below to request an appointment. Keep in mind that your wellness is our top priority. Regular wellness visits and appointments for chronic disease management are important for your health and should not be avoided.
