Important Information for Crestpoint Medicare Members

As you may be aware, Crestpoint Medicare will no longer be an option for your Medicare coverage after May 31, 2016. You should have received a letter from Crestpoint outlining your options. You may either:

1. Select another Medicare Advantage product with enhanced benefits
2. Be automatically enrolled in Traditional Medicare for your health coverage

State of Franklin Healthcare and all of our providers participate with these Medicare Plans:

• United Healthcare Medicare (also known as SecureHorizons)
• BCBS Medicare Advantage PPO and HMO
• Humana Medicare
• Traditional Medicare

To assist you with this transition, SoFHA will have insurance agents from United Healthcare Medicare available in our office buildings beginning April 25, 2016 through early June. An agent will be available Monday, Wednesday and Friday between 10:00 a.m. – 12:00 p.m. and 2:00 p.m. – 4:00 p.m. We are also working with Humana Medicare agents to arrange a schedule to be available; however, it has not been finalized.
