Back-to-School Health Checklist for Children & Teens


School is already back in session across the region, and at SOFHA, we know this time of year is filled with excitement for both kids and parents. A healthy start can make all the difference in helping your child feel confident, focused, and ready to thrive in the classroom.

Here’s our SOFHA-approved checklist to help your family start the school year strong.

1. Schedule a Well-Child Visit

Your child’s annual physical is the perfect time to check in on their growth, development, and overall wellness.

  • We’ll review nutrition, sleep, screen time, and answer age-appropriate questions about puberty.
  • If your child plays sports, we can complete their sports physical at the same time.

2. Stay Up to Date on Vaccinations

School immunization requirements can vary, so be sure your child is current on all recommended and required vaccines. Our team can review their record and make sure they’re protected.

3. Vision & Hearing Screenings

Children often don’t realize when they have trouble seeing or hearing.

  • Schedule a screening if they complain of headaches, squint, or if teachers notice difficulty focusing or listening.
  • Early detection helps prevent learning setbacks.

4. Dental Check-Up

Healthy smiles matter for confidence and overall health.

  • Aim for a cleaning and exam every 6 months.
  • Address any cavities, orthodontic needs, or sealant applications.
  • Keep up daily brushing and flossing habits.

5. Mental Health & Emotional Check-In

A new school year can bring excitement—but also stress, anxiety, or peer pressure.

  • Talk openly about school, friendships, and academics.
  • Watch for changes in mood, sleep, appetite, or energy.
  • If needed, we can connect you with a counselor or mental health specialist.

6. Nutrition & Meal Planning

Good nutrition fuels better learning.

  • Start the day with a protein-rich breakfast.
  • Pack balanced snacks and encourage plenty of water.
  • Teach lunchbox safety, especially with perishable items.

7. Sleep Routine Reset

Getting enough rest is key for focus, mood, and growth.

  • Begin adjusting bedtimes and wake-up times 1–2 weeks before school starts.
  • Kids (6–12 years): 9–12 hours of sleep per night
  • Teens (13–18 years): 8–10 hours of sleep per night

8. Hygiene Habits Refresher

Healthy habits help prevent illness from spreading in the classroom.

  • Wash hands often—especially before eating and after using the restroom.
  • Cough or sneeze into elbows.
  • Avoid sharing food, drinks, or personal items.

9. Update Health Forms & Medications at School

Make sure your child’s school nurse has everything they need:

  • Allergy details and medication instructions
  • Emergency contacts
  • Action plans for asthma, diabetes, or other chronic conditions
